Coronavirus Small Business Loan Update by Chamber Of Commerce - 2020-03.pdf (open link) copy and paste into your web browser...

 Applicants should be prepared to submit the following:

 i.     Completed SBA loan application (SBA Form 5 or 5C)

 ii.     Tax Information Authorization (IRS Form 4506T) for the business, principals and affiliates

iii.     Complete copies of the most recently filed Federal Income Tax Return

iv.     Schedule of Liabilities (SBA Form 2202)

v.     Other information may be requested

As of yesterday, SBA’s Disaster Loan Assistance Portal https://disasterloan.sba.gov/ela was down for maintenance.  They did have a webpage https://disasterloan.sba.gov/ela/Home/OfflineApply where you could download, fill out and upload your filled out the loan application forms.
